Roles & Responsibilities

• Evaluate patients and develop individualized treatment plans that align with physician orders and patient goals
• Deliver skilled physical therapy interventions across inpatient, swing bed, outpatient, and skilled nursing settings
• Document care accurately and timely in the EMR to support continuity and quality of care
• Use strong clinical reasoning to adjust treatments and progress patients toward functional goals
• Collaborate closely with physicians, nurses, and interdisciplinary team members to coordinate patient care
• Educate patients and families on home programs, safety, and techniques to maximize recovery
• Participate in ongoing professional development and quality improvement activities to advance clinical practice

Qualifications

• Active California Physical Therapy License in good standing
• Successful completion of the National Physical Therapy Examination (NPTE)
• Current AHA Basic Life Support (BLS) certification
• Bachelor's, Master's, or Doctor of Physical Therapy (DPT) from an accredited program
• Minimum 1 year experience in acute care, swing bed/skilled nursing, or outpatient settings
• Proficiency with EMR systems and basic computer skills

Education

• Bachelor's, Master's, or Doctor of Physical Therapy (DPT) from an accredited program

Knowledge & Skills

• Patient evaluation and treatment planning across diverse diagnoses
• Clinical reasoning and strong documentation practices
• Acute care and swing bed/skilled nursing experience
• Excellent communication and interdisciplinary collaboration
• Problem-solving, attention to detail, and ability to manage diverse patient populations
• Familiarity with EMR systems and digital charting
