Roles & Responsibilities

• Perform timely order entry and clinical verification to ensure medication safety
• Check and dispense medications accurately for inpatient populations
• Provide comprehensive patient care across age groups from geriatrics to pediatrics
• Rotate into ambulatory services when scheduled, such as anticoagulation clinic or oncology
• Collaborate with providers, nurses, pharmacy technicians, and the care team to optimize therapy
• Support medication-use policies and ensure compliance with state and federal pharmacy laws
• Deliver patient education and counseling to promote adherence and positive outcomes

Qualifications

• Current Pennsylvania pharmacist license or eligible for reciprocation within 90 days
• Bachelor of Science in Pharmacy required; Doctor of Pharmacy preferred
• Recent hospital pharmacy experience preferred
• Pharmacy residency and advanced practice certifications preferred for ambulatory rotations
• BLS certification required for certain clinic rotations and renew every two years
• Strong communication skills and ability to work with multidisciplinary teams

Education

• Bachelor of Science in Pharmacy (required)
• Doctor of Pharmacy (PharmD) (preferred)

Knowledge & Skills

• Medication order verification and accurate dispensing practices
• Inpatient pharmacy workflows and ambulatory pharmacy services
• Knowledge of state and federal pharmacy laws and regulatory requirements
• Patient-centered care skills across diverse populations
• Ability to collaborate with clinical teams and medical staff
• Comfort in anticoagulation and oncology clinic processes when assigned
