Roles & Responsibilities

• Provide skilled occupational therapy services in patients' homes with compassion and clinical rigor
• Complete OASIS assessments and accurate charting in EPIC to support continuity of care
• Develop individualized treatment plans focused on activities of daily living and functional independence
• Perform hands-on interventions and use appropriate modalities to address orthopedic and soft tissue needs
• Educate patients and caregivers on safe home strategies, adaptive equipment, and fall prevention
• Collaborate with interdisciplinary team members and communicate timely updates on patient progress
• Manage your schedule efficiently to maximize patient visits while minimizing drive time

Qualifications

• Licensed Occupational Therapist (OT) in Oregon or pending license accepted
• Minimum 1 year of skilled home health OT experience required - first-timers not accepted
• BCLS certification required
• OASIS experience required; please call out if you do not have OASIS experience
• Charting experience with EPIC required
• Facility requires primary COVID vaccination series plus one booster
• Reliable transportation required; occasional drive radius may be 50+ miles
• Locals accepted within a 50-75 mile radius

Knowledge & Skills

• Skilled in Home Health delivery and in-home safety assessments
• Proficient with OASIS and EPIC charting systems
• Strong expertise in activities of daily living (ADLs) training and adaptive techniques
• Familiarity with modalities and manual therapies: TENS, ultrasound, electrical stimulation, edema massage
• Orthopedic and post-operative rehab experience (knee, hip, soft tissue injuries)
• Experience with prosthetics/orthotics and splinting techniques
• Comfort working with adult and pediatric populations as needed
• Good time management and route planning to minimize windshield time
