Roles & Responsibilities

Provide compassionate direct patient care and conduct routine assessments
Administer medications and treatments safely and accurately
Document patient status, care delivered, and any changes in condition
Collaborate with nurses, therapists, and care team members to coordinate care
Communicate clearly and kindly with patients and families about care plans
Support infection control and safety protocols to maintain a healthy environment

Qualifications

Current LPN license in Pennsylvania (or compact/state-appropriate licensure if applicable)
Active Basic Life Support (BLS) certification or willingness to obtain
Comfortable working 8-hour shifts and delivering hands-on resident care
Strong communication skills and ability to work as part of a multidisciplinary team
Proof of required immunizations and background checks as facility policy requires

Education

LPN diploma or vocational nursing certificate
High school diploma or equivalent

Knowledge & Skills

Medication administration and basic nursing procedures
Patient assessment and vital sign interpretation
Skilled charting and documentation practices
Basic wound care and infection prevention techniques
Ability to use electronic health record systems
Compassionate bedside manner and strong teamwork abilities
