Roles & Responsibilities

• Provide compassionate direct patient care consistent with LPN scope of practice
• Administer medications and treatments as ordered and monitor patient response
• Assess and document patient vital signs, conditions, and progress with accuracy
• Assist residents with activities of daily living and promote dignity and comfort
• Collaborate with RNs, CNAs, and interdisciplinary team members for coordinated care
• Maintain accurate charting and ensure compliance with facility policies and regulations
• Communicate clearly and empathetically with residents and their families
• Participate in quality improvement initiatives and safety practices

Qualifications

• Current, active LPN license for the state of Connecticut (required)
• Basic Life Support (BLS/CPR) certification (or willingness to obtain)
• Ability to pass background check and drug screening as required by the facility
• Excellent communication and interpersonal skills
• Previous experience in long-term care or skilled nursing preferred but not required
• Dependable, compassionate, and team-oriented attitude

Education

• Graduate of an accredited Practical Nursing (LPN/LVN) program

Knowledge & Skills

• Fundamentals of medication administration and documentation
• Basic patient assessment and monitoring of vital signs
• Familiarity with infection control and safety best practices
• Comfortable assisting with activities of daily living and mobility support
• Competence with electronic health record charting (or willingness to learn)
• Good time management and prioritization in a fast-paced care setting
