Roles & Responsibilities

Provide compassionate direct patient care consistent with LPN scope
Administer medications and treatments safely and accurately
Monitor patient condition and report changes to the RN or provider
Document assessments, interventions and patient responses in the chart
Support activities of daily living and promote patient comfort
Collaborate with the interdisciplinary team to deliver coordinated care
Adhere to infection control and facility policies to ensure a safe environment

Qualifications

Valid LPN license (Connecticut) required
Current basic life support certification as required by facility policy
Comfortable working in a long-term care or skilled nursing environment
Strong communication and teamwork skills to partner with patients and staff

Education

LPN diploma or certificate from an accredited program

Knowledge & Skills

Skilled in basic nursing assessments and monitoring
Medication administration and documentation
Familiarity with long-term care or skilled nursing workflows
Patient-centered care and strong interpersonal skills
Basic electronic charting experience preferred
