Roles & Responsibilities

• Provide compassionate direct care to residents, assisting with activities of daily living (bathing, dressing, grooming, feeding)
• Support rehabilitation activities under direction of nursing staff and therapists
• Observe and report changes in resident condition to the nursing team promptly
• Accurately document care, observations and interventions in resident records
• Maintain a safe, clean environment and follow infection control and safety procedures
• Communicate clearly and courteously with residents, families and interdisciplinary team members to support excellent care

Qualifications

• Current LNA/T-LNA license (appropriate for VT/NH assignments) — must be active and in good standing
• Availability to start the travel assignment beginning 2026-04-20 through 2026-07-20
• Weekend availability — every other weekend required as noted
• Reliable transportation to and from the facility

Knowledge & Skills

• Hands-on resident care skills and comfort with ADLs
• Good documentation and charting practices
• Strong communication and teamwork skills
• Ability to follow care plans and directions from nursing/therapy staff
• Time management and ability to prioritize during busy shifts
