Shift Type: Monday-Friday, 8:30 AM - 5:00 PM, with rotating weekends and holidays per facility policy

Candidate Pay Rate: $53 per hour

Roles & Responsibilities

• Coordinate comprehensive patient care and discharge planning
• Utilize Meditech and Midas for accurate documentation and case management
• Collaborate with multidisciplinary team to ensure optimal patient outcomes
• Assess patient needs and develop individualized care plans
• Facilitate communication between patients, families, and healthcare providers

Qualifications

• Active RN license in Florida or compact state
• Minimum 1 year of case management experience required
• Experience with Meditech and Midas is mandatory
• BLS certification from American Heart Association
• Strong complex discharge planning skills preferred

Knowledge & Skills

• Excellent communication and interpersonal skills
• Ability to work independently and in a team
• Proficient in electronic medical records (Meditech, Midas)
• Knowledge of discharge planning and resource coordination
• Adaptability to float between units as needed
