Roles & Responsibilities

• Deliver personalized physical therapy care to patients in the comfort of their own homes
• Create customized treatment plans that focus on restoring mobility, strength, and independence
• Collaborate with physicians, nurses, and other healthcare professionals to ensure comprehensive care
• Educate patients and their families on therapeutic techniques and fall prevention strategies
• Maintain accurate and timely documentation of patient progress and outcomes
• Adapt treatment approaches to meet the unique needs and environments of each home setting

Qualifications

• Active Physical Therapist (PT) license in the state of Iowa
• Current BLS certification (required)
• Experience in home health or outpatient settings is a plus
• Comfortable working independently and managing a flexible daily schedule
• Epic EHR experience preferred, but not required

Knowledge & Skills

• Strong clinical assessment and treatment planning skills
• Excellent communication and patient education abilities
• Time management and organizational skills to manage a diverse caseload
• Knowledge of home health safety standards and infection control practices
• Adaptability to work in various home environments
• Basic computer skills for electronic documentation
