Roles & Responsibilities

• Conduct comprehensive clinical screening, assessment, diagnosis, treatment planning and brief therapy for clients from birth through elder
• Lead and coordinate inpatient discharge planning and arrangements for ALF or transitional placements
• Facilitate or co-facilitate patient care conferences and collaborate with multidisciplinary teams
• Provide care coordination, client advocacy, and skill-building, including assistance with complex matters such as Power of Attorney
• Offer clinical support and appropriate referrals for chemical dependency and mental health needs
• Maintain accurate, timely, and compliant documentation to meet state, federal, accreditation, and hospital billing standards
• Complete required provider enrollment documentation and assigned billing duties in a timely manner
• Participate in staff trainings and quality management activities to integrate best practices into service delivery

Qualifications

• Master's degree in Social Work, Psychology, Counseling or related discipline from an accredited institution
• Licensed as or in the process of becoming a Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), or Licensed Marriage and Family Therapist (LMFT)
• Valid State of Alaska driver’s license or ability to obtain within 6 months of hire
• At least 21 years of age
• Clinical competency must be achieved within 3 months of hire and maintained every three years

Education

• Master's Degree in Social Work, Psychology, Counseling, or related discipline

Knowledge & Skills

• Discharge planning and care coordination
• Clinical assessment, screening, diagnosis, and treatment planning
• Brief therapy and continuing care across the lifespan
• Crisis intervention and interviewing techniques
• Accurate clinical documentation and knowledge of accreditation and billing requirements
• Experience working with tribal health, cultural wellness, and multidisciplinary teams
• Familiarity with electronic health records and provider enrollment processes
• Case management and client advocacy skills
