Location: Louisville, Kentucky

Start Date: 06-22-2026

End Date: 09-21-2026

Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with rotating weekend shifts 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Provide comprehensive physical therapy services in home health settings
Conduct start of care assessments, recertifications, and evaluations
Deliver daily visits to patients with CHF, orthopedic conditions, and pneumonia
Develop personalized treatment plans for diverse patient populations
Collaborate with clinical managers and educators for optimal patient outcomes
Document patient progress and maintain accurate clinical records

Qualifications

Current Physical Therapist license required
BLS certification required
Home health experience preferred but not required
No new grads - experienced clinicians preferred
Strong communication and documentation skills

Knowledge & Skills

Home health physical therapy expertise
Patient assessment and evaluation skills
Treatment planning and progress documentation
Comfortable working independently in the field
Excellent time management and organizational abilities
