Roles & Responsibilities
- Conduct comprehensive OASIS assessments for new patients
- Manage start of care visits with compassion and expertise
- Provide skilled physical therapy services across four rural counties
- Track patient progress using tablet-based documentation
- Help elderly patients recover from falls and fractures
- Build meaningful relationships with patients in their homes
Qualifications
- Active Physical Therapist license in Indiana
- Experience with OASIS assessments preferred
- Comfortable working independently in rural settings
- Strong communication and documentation skills
- Valid driver's license and reliable transportation
