Location: Louisville, Kentucky

Start Date: 06-22-2026

End Date: 09-21-2026

Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with rotating weekend shifts 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Provide comprehensive physical therapy services including start of care, recertifications, evaluations, and daily visits
Deliver compassionate care to patients with CHF, orthopedic conditions, and pneumonia in their homes
Collaborate with clinical managers and educators to ensure exceptional patient outcomes
Complete thorough documentation and care coordination for optimal patient recovery
Build meaningful relationships with patients while helping them regain independence and mobility

Qualifications

Current Physical Therapist license in Kentucky
BLS certification required
Home health experience preferred but not required
Strong communication and clinical assessment skills
Valid driver's license and reliable transportation

Knowledge & Skills

Home health physical therapy expertise
Patient assessment and care planning
Electronic documentation systems
Time management and independent decision-making
Compassionate bedside manner and patient education
