Roles & Responsibilities

Conduct OASIS assessments and Start of Care visits for new patients
Provide personalized physical therapy to patients recovering from falls and fractures
Travel across four rural counties to deliver care where it's needed most
Document visits and track progress using tablet technology
Collaborate with a multidisciplinary team to support patient recovery goals

Qualifications

Active Physical Therapist license in Indiana
Experience with OASIS assessments and Start of Care documentation
Comfortable working independently in rural settings
Strong communication and time-management skills

Knowledge & Skills

OASIS assessment expertise
Start of Care visit experience
Tablet-based documentation
Rural community healthcare delivery
