Candidate Pay Rate: $53 per hour

Roles & Responsibilities

Conduct OASIS assessments and Start of Care visits for new patients
Provide skilled physical therapy services across four rural counties
Help patients recover from falls, fractures, and age-related mobility challenges
Document visits and track time using facility tablet system
Manage a manageable caseload of 4-6 patient visits daily

Qualifications

Current Physical Therapist license in Indiana
Experience with OASIS assessments and Start of Care documentation
Comfortable working independently in rural settings
Strong communication and time-management skills

Geriatric rehabilitation
