Candidate Pay Rate: $58 per hour

Roles & Responsibilities

Provide compassionate physical therapy services to patients in their homes across Yolo County
Assess and evaluate each patient's therapeutic needs and develop personalized care plans
Guide patients and families through exercises and the safe use of therapeutic equipment
Monitor progress and communicate updates with physicians to ensure optimal outcomes
Train and supervise home health aides participating in rehabilitation programs
Document clinical notes and progress summaries to support quality patient care

Qualifications

Minimum 2 years of physical therapy experience in acute, home care, or rehab settings
Current BLS certification required
Strong communication and organizational skills
Valid driver's license and reliable transportation for home visits
Two supervisory references from your modality (one within 12 months, one within 3 years)

Knowledge & Skills

Home health physical therapy assessment and treatment planning
Patient and family education on therapeutic exercises and equipment
Understanding of Medicare, Medi-Cal, and Title 22 regulations
Ability to work independently while collaborating with interdisciplinary teams
Strong documentation and time-management abilities
