Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Provide compassionate physical therapy services to patients in their homes across a 60-mile radius
Help patients recover from surgeries, falls, and other conditions with personalized treatment plans
Support both Home Health and Hospice patients on their wellness journeys
Document care using HomeCare/HomeBase EMR system
Build meaningful relationships while delivering therapy that truly matters

Qualifications

Current North Dakota Physical Therapy License required
BLS certification needed
At least 1 year of physical therapy experience with previous home health background
Valid driver's license and auto insurance essential
Strong PT-related clinical skills

Knowledge & Skills

Home health physical therapy expertise
Experience with post-surgical and fall recovery patients
Comfortable working independently in home settings
Familiarity with HomeCare/HomeBase EMR preferred
Excellent communication and time-management abilities
