Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Deliver personalized physical therapy services to patients recovering from surgery, falls, and other conditions in their homes
Assess each patient's unique needs and create tailored treatment plans that promote independence and healing
Collaborate with home health and hospice teams to ensure seamless, compassionate care
Educate patients and families on exercises and safety techniques to support recovery between visits
Document progress accurately using HomeCare/HomeBase EMR to keep the care team informed

Qualifications

Current North Dakota Physical Therapy License
BLS certification required
At least 1 year of previous home health experience
Valid driver's license and auto insurance for travel within the service area
Strong PT clinical skills and a passion for patient-centered care

Knowledge & Skills

Home health physical therapy expertise
Comfortable managing a caseload of 5-7 patients per day
Familiarity with HomeCare/HomeBase EMR system
Excellent communication and patient education skills
Ability to work independently within a 60-mile radius of Valley City
