Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Provide compassionate physical therapy services to home health and hospice patients
Help patients recover from surgeries, falls, and other conditions in the comfort of their homes
Develop personalized treatment plans that support each patient's unique goals
Travel within a 60-mile radius around Valley City to deliver care where it's needed most
Document patient progress using the HomeCare/HomeBase EMR system
Collaborate with interdisciplinary teams to ensure seamless patient care

Qualifications

Current North Dakota Physical Therapy License required
BLS certification needed
At least 1 year of physical therapy experience
Previous home health experience strongly preferred
Valid driver's license and auto insurance required
Must not have worked full-time, part-time, or PRN at any CommonSpirit, CHI, or Dignity facility in the past 12 months

Knowledge & Skills

Strong clinical skills in physical therapy assessment and treatment
Comfortable working independently in home settings
Excellent communication and patient education abilities
Familiarity with HomeCare/HomeBase EMR is a plus
Ability to manage a 5-7 patient daily caseload efficiently
