Location: Santa Maria, California
Start Date: 06-01-2026
End Date: 08-31-2026
Contract Type: Contract
Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with every 3rd weekend rotation
Hours per Week: 40
Candidate Pay Rate: $64 per hour

Roles & Responsibilities
- Deliver personalized physical therapy services directly in patients' homes across your assigned region
- Evaluate and treat a diverse caseload including orthopedic conditions, stroke recovery, joint replacements, and spinal injuries
- Complete OASIS documentation to ensure compliance with government-mandated requirements
- Collaborate with an interdisciplinary team of PTs, OTs, and Speech Therapists to optimize patient outcomes
- Build meaningful relationships with patients while helping them regain independence and mobility
Qualifications
- Active Physical Therapist license in California
- Current BLS certification required
- Minimum one year of clinical experience as a Physical Therapist
- Experience with OASIS documentation required
- Home health experience preferred but not required
- Cerner EMR experience a plus
Knowledge & Skills
- Strong assessment and treatment planning skills for home-based care
- Excellent communication and patient education abilities
- Comfortable working independently in diverse home environments
- Proficient with OASIS and Cerner EMR documentation
- Flexible and adaptable approach to patient care
