Location: Santa Maria, California
Start Date: 05-30-2026
End Date: 08-29-2026
Contract Type: Contract
Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with every 3rd weekend rotation
Hours per Week: 40
Candidate Pay Rate: $64 per hour

Roles & Responsibilities
- Deliver personalized physical therapy services in patients' homes across your assigned region
- Evaluate and treat a diverse caseload including ortho, stroke, joint replacement, and spinal injury patients
- Complete OASIS documentation to ensure compliance with government requirements
- Collaborate with OT, Speech Therapy, and rehab aides to provide comprehensive care
- Educate patients and families on home exercise programs and recovery strategies
- Maintain accurate clinical records using Cerner EMR
Qualifications
- Active Physical Therapist license in California
- At least one year of experience as a Physical Therapist
- Home health experience preferred but not required
- BLS certification required
- Proficiency with OASIS documentation
- Cerner EMR experience is a plus

- Strong assessment and clinical reasoning skills
- Excellent communication and patient education abilities
- Comfortable working independently in home settings
- Organized with attention to documentation accuracy
- Flexible and adaptable to varied patient needs
