Candidate Pay Rate: $53 per hour

Roles & Responsibilities

Conduct Oasis Assessments and Start of Care cases for patients in a home health setting
Manage a caseload of 4-6 patient visits daily using a point system
Provide physical therapy services to elderly patients recovering from falls and fractures
Serve patients across four counties: Noble, Lagrange, Steuben, and DeKalb
Utilize a tablet to track time and visits efficiently

Qualifications

Current Physical Therapy license in Indiana
Experience with Oasis Assessments and Start of Care required
Comfortable working in a rural home health environment

Knowledge & Skills

Proficient in Oasis Assessments and Start of Care documentation
Strong clinical skills in geriatric physical therapy
Ability to manage a point-based patient visit schedule
Excellent communication and time management skills
