Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Provide compassionate physical therapy services to patients in their homes
Conduct start of care, recertifications, and resumption of care visits
Complete 6.5 points per day, balancing regular visits and admissions
Collaborate with clinical coordinators and fellow therapists for support
Perform safety evaluations and occasional hospice teaching visits
Document care using OASIS and maintain accurate patient records

Qualifications

Current ND Physical Therapy License
BLS certification
Valid driver's license and auto insurance
Home Health experience preferred
OASIS training preferred
First-time travelers welcome

Home Health physical therapy expertise
OASIS documentation skills
Strong communication and time management
Ability to work independently in the field
