Roles & Responsibilities

• Provide skilled physical therapy services within your legal scope of practice
• Conduct initial evaluations, progress notes, and recertifications per Medicare and agency standards
• Develop and update individualized plans of care in collaboration with physicians
• Supervise Physical Therapist Assistants and Home Health Aides as per state regulations
• Educate patients and caregivers on home exercise programs and adaptive strategies
• Prepare and submit OASIS assessments and documentation accurately and on time
• Apply ICD-10 coding for compliant documentation
• Participate in QAPI activities and contribute to continuous improvement

Qualifications

• Graduate of an accredited physical therapist curriculum (CAPTE, APTA, or CAAHEP approved)
• Currently licensed as a Physical Therapist in New Mexico
• Current CPR certification
• Two years of clinical experience preferred; home health experience a plus
• Reliable transportation, valid driver's license, and auto insurance for home visits
• Ability to work independently and exercise sound clinical judgment

Education

• Graduate of an accredited physical therapist curriculum approved by the Commission on Accreditation in Physical Therapy Education, the American Physical Therapy Association, or the Committee on Allied Health Education and Accreditation

Knowledge & Skills

• Proficient in OASIS documentation and ICD-10 coding
• Comfortable with EMR systems and basic computer applications
• Strong communication and interpersonal skills
• Knowledge of Medicare/Medicaid Conditions of Participation
• Ability to adapt to changing patient needs and work settings
