Roles & Responsibilities

• Assess patient function and develop individualized treatment plans focused on meaningful outcomes
• Deliver therapeutic interventions including exercises, gait training, and manual techniques
• Document progress clearly and timely in the patient record to support continuity of care
• Collaborate with interdisciplinary teams to coordinate discharge planning and maximize recovery
• Educate patients and families on home programs and injury prevention to support long-term gains
• Use clinical reasoning to modify treatments and optimize patient independence

Qualifications

• Licensed Physical Therapist in New Hampshire or eligible to obtain state licensure
• Doctor of Physical Therapy (DPT) or Master's in Physical Therapy preferred
• Current BLS/CPR certification (or willingness to obtain promptly)
• Comfortable working in a fast-paced, goal-oriented rehabilitation environment
• Strong communication and teamwork skills; experience with EMR documentation a plus
• Previous outpatient/contract or travel PT experience preferred but not required

Education

• Doctor of Physical Therapy (DPT) or Master of Physical Therapy (MPT)
• Bachelor’s degree acceptable with relevant advanced PT credentials in some cases

Knowledge & Skills

• Skilled in assessment and therapeutic exercise prescription
• Experience with mobility training, balance and fall prevention strategies
• Proficiency in manual therapy techniques and adaptive equipment recommendations
• Strong clinical documentation and outcome tracking
• Ability to collaborate with nursing and rehab team members to coordinate care
• Patient education and family-centered care approach
