Location: Concord, CA

Candidate Pay Rate: $55 - $65 per hour

Roles & Responsibilities

Provide physical therapy treatments under the supervision of a licensed Physical Therapist to help residents regain function
Assist residents with therapeutic exercises and mobility training to improve independence
Implement individualized treatment plans tailored to each resident's goals
Monitor and accurately document patient progress to support continuity of care
Collaborate with interdisciplinary healthcare teams to coordinate excellent patient-centered care
Ensure compliance with facility and state regulations to maintain a safe, high-quality environment

Qualifications

Active California PTA License (required)
Current BLS Certification
SNF experience preferred
Strong clinical judgment and excellent communication skills

Knowledge & Skills

Physical therapy treatments and modalities
Therapeutic exercise design and progression
Mobility training and fall prevention strategies
Developing and implementing individualized treatment plans
Accurate patient progress monitoring and documentation
Team-based collaboration in a skilled nursing setting
