Roles & Responsibilities

• Provide skilled occupational therapy in patients' homes, delivering meaningful care that helps people regain independence
• Complete timely OASIS assessments and chart accurately in EPIC to ensure continuity of care and compliance
• Manage a focused caseload (may visit up to six patients per day) while maintaining quality and patient-centered goals
• Develop and implement individualized treatment plans using modalities and manual therapy to address ADLs and functional needs
• Collaborate with nurses, aides, and other clinicians to coordinate care and communicate progress
• Educate patients and caregivers on safe home strategies, adaptive equipment, and activity progression
• Maintain required certifications and adhere to facility vaccine and safety policies

Qualifications

• Minimum 1 year of skilled home health occupational therapy experience required
• Valid Occupational Therapist license for the state of practice; pending license accepted
• BCLS certification required
• Experience with OASIS is required; please note if you do not have OASIS experience
• Charting experience in EPIC preferred or required based on facility notes
• Reliable transportation for travel to patient homes, ability to manage occasional drives up to and beyond a 50 mile radius

Knowledge & Skills

• Home Health clinical skills including Activities of Daily Living assessment and intervention
• Proficiency with OASIS documentation and EPIC charting workflows
• Modalities and manual therapy skills such as TENS, ultrasound, electrical stimulation, and edema massage
• Orthopedics knowledge including hip and knee replacements, soft tissue injuries, and hand injuries
• Prosthetics and orthotics familiarity including dynamic and static splints
• Pediatric and acute care exposure a plus, as well as cardiac rehab and feeding techniques
• Strong communication, time management, and patient education skills for community-based care
