Roles & Responsibilities

• Administer medications and treatments accurately while monitoring patient responses
• Perform thorough patient assessments and document vital signs and health status
• Collaborate with nurses, physicians, and the care team to deliver comprehensive care
• Provide compassionate hands-on care including wound care, tube feedings, and catheter care
• Educate patients and families on care plans, medications, and health management
• Maintain a safe, clean, and supportive environment for residents and patients

Qualifications

• Active Licensed Practical Nurse (LPN) license in Pennsylvania
• Current BLS/CPR certification required
• Strong clinical assessment and critical thinking skills
• Prior experience in long-term care or skilled nursing preferred
• Ability to work flexible shifts including nights and weekends

• Medication administration and dosage calculations
• Electronic health records (EHR) documentation
• Patient assessment and care planning
• Clear communication and interpersonal skills
• Infection control and safety protocols
• Time management and prioritization in fast-paced environments
