Roles & Responsibilities

Provide compassionate, hands-on care to residents at Pocahontas Center
Administer medications and monitor patient responses
Collaborate with the care team to support individualized treatment plans
Document patient progress and communicate updates effectively
Ensure a safe and supportive environment for every resident

Qualifications

Active LPN license in West Virginia
Current CPR certification
Strong clinical and communication skills
Ability to work independently and as part of a team

Knowledge & Skills

Medication administration and documentation
Patient assessment and vital signs monitoring
Infection control practices
Electronic health record systems
