Roles & Responsibilities

Provide nursing care to clinic patients within the scope of LPN licensure, following established standards and practices.
Act as the primary liaison between patients, clinical staff, and providers to ensure coordinated, high-quality care.
Perform vital signs assessment and triage patient calls to determine appropriate next steps and escalate when needed.
Manage medication refills and complete prior authorizations to keep patients on their therapies without unnecessary delays.
Support and direct CNAs and Schedulers in daily tasks, ensuring smooth clinic operations and patient flow.
Collaborate with providers, coordinators, clinical managers/supervisors, and RNs to meet patient needs and improve outcomes.

Qualifications

Current State LPN license (must be active and in good standing)
BLS (Basic Life Support) certification
Graduate of a Nursing Program
Comfortable taking vital signs, triaging patient calls, and managing medication refills/prior authorizations
Ability to delegate and supervise CNAs and work closely with schedulers and interdisciplinary teams

Education

Graduate of a Nursing Program

Knowledge & Skills

Clinical nursing skills and patient assessment/triage
Medication management including refills and prior authorizations
Strong communication and liaison abilities with patients and providers
Experience working collaboratively within an interdisciplinary healthcare team
Scheduling coordination and day-to-day clinic operations support
Delegation and supervision of CNAs
