Roles & Responsibilities

• Interview patients and families to gather psychosocial information and assess needs
• Develop and participate in interdisciplinary plans of care to support patient goals
• Coordinate discharge planning and refer patients to home- and community-based services
• Provide crisis counseling, therapeutic support, and patient/family education
• Manage assigned caseload and document psychosocial assessments and treatment plans
• Handle referrals and consults from primary care and outlying clinics within scope
• Link patients to community resources and work with state/federal agencies for placement needs
• Advocate for patients, including those experiencing homelessness, addictions, or severe mental illness

Qualifications

• Active Alaska LCSW required at time of submittal (LMSW accepted where noted)
• Minimum 2-4 years of social work experience in inpatient or outpatient healthcare settings
• NABCS Clearance and ICPA Clearance required
• Flu vaccine required prior to start
• Must be at least 21 years of age
• Competence with electronic health records and general computer skills

Education

• Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW)

Knowledge & Skills

• Strong crisis intervention and mental health evaluation skills
• Excellent clinical documentation and treatment planning
• Case management, discharge planning, and care coordination
• Empathic listening, clear oral and written communication, and conflict resolution
• Familiarity with community resources and ability to coordinate with external agencies
• Comfort using computers, EHRs, Google Drive, fax and phone systems
• Basic nutrition and tobacco cessation knowledge to support physical health education
