Roles & Responsibilities

• Perform thorough patient evaluations to develop effective, individualized treatment plans
• Deliver skilled treatments focused on functional mobility, safety and discharge readiness
• Use safe patient handling equipment and rehab tools to provide confident, evidence-based care
• Document timely, accurate patient records in Meditech to support continuity of care
• Collaborate with nursing, therapy techs and interdisciplinary team members to optimize patient outcomes
• Educate patients and families on home programs, safety and recovery goals to empower lasting gains

Qualifications

• Licensed Physical Therapist with current state licensure
• Minimum 2 years of experience in acute care and/or inpatient rehab settings
• Comfortable with Meditech documentation or similar electronic medical record systems
• Experience using safe patient handling equipment and standard rehab modalities
• Strong communication skills and ability to work within a multidisciplinary team

Knowledge & Skills

• Proficient with Meditech documentation and clinical charting
• Skilled in inpatient rehab evaluations and treatment planning
• Familiarity with safe patient handling equipment and common rehab devices
• Ability to manage typical patient ratios and prioritize caseloads
• Flexible mindset to float between acute and rehab units when needed
