Roles & Responsibilities

• Provide comprehensive primary care to frail seniors, including diagnosis and treatment of chronic and acute illnesses, preventative care, and urgent medical needs
• Conduct thorough health assessments and evaluate patient needs in the clinic, the PACE Day Center, and patients' homes
• Collaborate closely with the entire PACE interdisciplinary team to deliver the highest quality, person-centered care
• Make appropriate and timely referrals to trusted contracted providers in the community to ensure seamless continuity of care

Qualifications

• Hold a current, unrestricted APN and RXN license in Colorado
• National certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP), or other appropriate population focus preferred
• Controlled substance license with current DEA registration or eligibility to obtain one
• Current CPR/BLS certification maintained throughout employment
• Valid driver's license, acceptable driving record, and proof of automobile insurance maintained throughout employment
• At least 2 years of experience as a Nurse Practitioner or in a related advanced practice role

Knowledge & Skills

• Skilled in primary and preventative care for older adults with complex, chronic conditions
• Confident managing both chronic and acute illnesses, as well as urgent care needs
• Strong health assessment and patient evaluation abilities across clinic and home settings
• A natural collaborator who thrives in an interdisciplinary care team
• Experienced at coordinating referrals and connecting patients with community resources
