Roles & Responsibilities

• Evaluate residents and create personalized therapeutic plans that focus on maximizing independence
• Provide ADL training, helping residents with bathing, dressing, and personal hygiene
• Assist with ambulation, mobility, and positioning, using mechanical lifts as appropriate
• Collaborate with physicians and the care team on therapeutic and discharge needs
• Document all evaluations and treatments in the EMR, ensuring accessible and clear records
• Educate residents and caregivers on therapeutic techniques and assistive devices

Qualifications

• Active Occupational Therapist (OT) license in the state of practice (PA per job description)
• BLS/BCLS certification required
• Prior experience in a long-term care or skilled nursing facility preferred
• Compassionate and resident-centered approach to care
• Strong communication and interpersonal skills to collaborate with an interdisciplinary team

Education

• Bachelor's degree in Occupational Therapy required
• Master's or Doctorate degree preferred

Knowledge & Skills

• Excellent verbal and written communication skills
• Strong interpersonal skills and a team-player mindset
• Analytical and sound decision-making abilities
• Proficient in Microsoft Office and EMR systems
• Excellent time management and documentation skills
• Ability to work collaboratively in an interdisciplinary team
