Roles & Responsibilities

• Provide direct patient care including assessing, monitoring, and documenting vital signs
• Administer medications and treatments as prescribed, ensuring accuracy and safety
• Collaborate with nurses and physicians to implement individualized care plans
• Assist patients with daily activities such as bathing, dressing, and eating
• Educate patients and families on health conditions and post-care instructions
• Maintain a clean and safe environment in accordance with facility policies

Qualifications

• Current LPN license in the state of Pennsylvania
• CPR/BLS certification required
• Graduate of an accredited practical nursing program
• Prior experience in a skilled nursing or long-term care setting is a plus
• Must be able to pass a background check and have valid identification

Knowledge & Skills

• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal abilities
• Proficiency in electronic health records (EHR) and documentation
• Ability to work independently and as part of a multidisciplinary team
• Compassionate and patient-centered approach to care
