Roles & Responsibilities

• Conduct comprehensive assessments to create individualized treatment plans
• Implement therapeutic exercises and interventions to improve mobility and function
• Collaborate with interdisciplinary teams to ensure holistic patient care
• Document patient progress and adjust treatment plans as needed
• Educate patients and families on home exercise programs and fall prevention

Qualifications

• Current California Physical Therapist (PT) license or eligibility
• Doctor of Physical Therapy (DPT) degree
• New graduates welcome; no prior experience required
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team

Education

• Doctor of Physical Therapy (DPT) degree

Knowledge & Skills

• Knowledge of geriatric rehabilitation and SNF care
• Proficiency in creating and executing treatment plans
• Skilled in patient assessment and documentation
• Familiarity with electronic health records (EHR) is a plus
