Roles & Responsibilities

  • Provide comprehensive occupational therapy services to patients in their homes, focusing on improving daily living skills and independence
  • Conduct thorough OASIS assessments to develop individualized care plans
  • Collaborate with patients, families, and interdisciplinary teams to set realistic goals and track progress
  • Educate patients and caregivers on adaptive techniques and home safety strategies
  • Document patient progress accurately and timely to ensure compliance with home health regulations
  • Travel to patient homes within a 35-50 mile radius, managing your schedule efficiently

Qualifications

  • Active Michigan LARA Occupational Therapy license (required)
  • 2+ years of current Occupational Therapy experience
  • 2+ years of current Home Health experience
  • OASIS experience required
  • Active American Heart Association BLS certification
  • Must have reliable personal vehicle with proof of auto insurance and a clean driving record
  • Residence must be more than 50 miles from the facility

Knowledge & Skills

  • Expertise in Occupational Therapy assessment and treatment planning
  • Strong knowledge of Home Health regulations and OASIS documentation
  • Excellent communication and interpersonal skills for patient and family education
  • Ability to work independently and manage a travel schedule effectively
  • Time management and organizational skills to prioritize patient visits
