Roles & Responsibilities

• Partner with residents and care providers to deliver physical therapy services that maximize functional abilities and independence
• Perform evaluative and therapeutic procedures under physician orders while observing precautions, indications, and contraindications
• Evaluate residents and create individualized treatment plans with functional short- and long-term goals
• Collaborate with residents, caregivers, physicians, and the healthcare team on plans of care and discharge needs
• Monitor treatment effectiveness and adjust plans to help residents reach their goals
• Document all evaluations and treatments in the Electronic Medical Record for seamless team communication
• Recommend and train residents on assistive devices to support daily living
• Supervise and mentor assistant-level staff and provide in-service education to other departments
• Ensure compliance with Medicare, Medicaid, and other reimbursement regulations

Qualifications

• Active Physical Therapist (PT) license in the applicable state of work
• 3+ years of physical therapy experience with skilled nursing experience required
• BLS/BCLS certification required
• Prior experience in a long-term care or skilled nursing facility preferred
• A compassionate, resident-centered approach to care

Education

• Bachelor's degree required
• Master's or Doctorate preferred

• Strong verbal, written, and presentation communication skills
• Excellent interpersonal and customer service skills
• Ability to work collaboratively in an interdisciplinary team
• Solid time management and documentation skills
• Compassionate and resident-centered care approach
• Proficient in Microsoft Office
• Sound analytical and decision-making abilities
• Ability to speak, read, write, and understand written instructions in English
