Roles & Responsibilities
- Collaborate with oncologists to establish and optimize supportive care measures for patients receiving chemotherapy, both inpatient and outpatient.
- Conduct staff development programs and educational sessions for pharmacy, nursing, and medical teams.
- Serve as the go-to oncology expert for drug information questions and ensure EHR care plans align with national guidelines.
- Round alongside oncology fellows, nurse practitioners, and physicians to optimize therapy using evidence-based guidelines and Bayhealth policies.
- Provide patient-specific medication education using teach-back methods for oral and intravenous chemotherapy.
- Review and update oncology care plans in the Electronic Health Record, ensuring appropriateness and accuracy.
- Identify, evaluate, and resolve medication-related problems based on clinical presentation, history, and lab data.
- Precept pharmacy residents and students, fostering the next generation of pharmacy leaders.
- Actively participate in oncology service line meetings to improve safety, efficacy, and quality of cancer care.
- Promote rational, cost-effective drug therapy and focus on cost avoidance within the oncology service line.
Qualifications
- Doctor of Pharmacy (PharmD) degree from an accredited program
- Current Delaware Pharmacist license (or eligibility to obtain)
- Board Certified Oncology Pharmacist (BCOP) credential
- PGY-1 residency or equivalent clinical pharmacy practice experience (at least 3 years)
- PGY-2 oncology residency or equivalent oncology practice experience (at least 3 years) preferred
- Minimum of 3 years of hospital experience with oncology training (5+ years preferred)
- Completion of ASHP-accredited residency(s) preferred
Education
- Doctorate Degree in Pharmacy
Knowledge & Skills
- Deep expertise in oncology pharmacy and pharmacotherapy
- Medication therapy management and chemotherapy protocols
- Skilled in using Electronic Health Records (EHR) for care planning
- Strong patient education abilities, including teach-back techniques
- Excellent drug information and evidence-based practice skills
- Ability to collaborate effectively with multidisciplinary healthcare teams
- Understanding of cost-effective, rational drug therapy
