Roles & Responsibilities

• Conduct clinical screenings, assessments, diagnoses, treatment planning, brief therapy, and continuing care for patients across the lifespan
• Lead and facilitate discharge planning and care coordination, including ALF/transitional placement planning
• Collaborate with hospital teams and community partners to connect patients to appropriate services and ensure smooth transitions of care
• Provide client advocacy, skill-building, and assistance with complex matters such as Power of Attorney when relevant
• Maintain accurate, timely, and compliant documentation to meet state, federal, accreditation, and hospital billing standards
• Support chemical dependency and mental health needs by providing clinical guidance and appropriate referrals
• Participate in quality management trainings and help integrate new practices into client care

Qualifications

• Master's degree in Social Work, Psychology, Counseling, or related discipline (required)
• Licensed or in process of becoming licensed as an LCSW, LPC, or LMFT
• Valid State of Alaska driver's license or ability to obtain within 6 months of hire
• Must be 21 years of age
• Clinical competency to be achieved within 3 months of hire and maintained every three years
• Experience in discharge planning, care coordination, case management, and clinical documentation

Education

• Master's Degree in Social Work, Psychology, Counseling, or related discipline from an accredited institution

Knowledge & Skills

• Discharge planning and care coordination best practices
• Interviewing techniques and crisis intervention
• Clinical assessment, diagnosis, treatment planning, and brief therapy
• Documentation standards for hospital billing, accreditation, and regulatory compliance
• Client advocacy and multidisciplinary team collaboration
• Familiarity with chemical dependency and mental health interventions
• Provider enrollment processes and timely completion of required reports
