Roles & Responsibilities

Provide compassionate, patient-centered hospice care wherever your patient calls home
Manage an assigned caseload and lead the interdisciplinary care team
Assess and manage symptoms with excellent clinical judgment and critical thinking
Educate patients and families to build confidence in caregiving
Document patient progress and collaborate with physicians and team members
Be the 'angel' that families remember, delivering strength and comfort

Qualifications

Active and unrestricted Registered Nurse (RN) license in Colorado
Minimum 2 years of nursing experience required; hospice or home health experience strongly preferred
Ability to travel to multiple patient locations within Grand Junction and surrounding areas
A passion for end-of-life care and making a meaningful difference
