Roles & Responsibilities
- Provide compassionate physical therapy services to patients in their homes according to physician care plans
- Assess and evaluate patients' therapeutic needs while developing personalized rehabilitation programs
- Guide patients and families through therapeutic exercises and proper use of assistive devices
- Monitor patient progress and communicate changes to physicians to optimize care outcomes
- Collaborate with home health aides and coordinate with other healthcare professionals for comprehensive patient support
- Document clinical progress and maintain accurate patient records to ensure continuity of care
Qualifications
- Minimum 2 years experience as a Physical Therapist in acute, home care, or rehab settings
- Current BLS certification required
- Strong oral and written communication skills with excellent organizational abilities
- Valid Physical Therapy license for California practice
- Ability to work independently while providing exceptional patient-centered care

- Home health rehabilitation techniques

- Gait analysis and mobility training
- Range of motion and muscle strength evaluation
- Prosthetic device training and adaptation
