Roles & Responsibilities
- Deliver personalized physical therapy care to patients in their homes
- Help patients recover from surgeries, falls, and other conditions with compassion
- Create individualized treatment plans that make a real difference
- Collaborate with home health and hospice teams to support whole-patient wellness
- Travel within a 60-mile radius to bring expert care directly to those who need it most
Qualifications
- Current North Dakota Physical Therapy License
- BLS certification
- At least 1 year of previous home health experience
- Valid driver's license and auto insurance
- Comfortable working independently in home settings
Knowledge & Skills
- Home health physical therapy expertise
- HomeCare/HomeBase EMR proficiency
- Strong clinical assessment and treatment planning skills
- Excellent communication and patient education abilities
- Ability to manage a caseload of 5-7 patients per day
