Hours per Week: 40
Candidate Pay Rate: $70 per hour

Roles & Responsibilities
- Provide compassionate physical therapy services to patients in their homes across Tehama County
- Conduct Start of Care visits especially for orthopedic patients when nursing is not ordered
- Develop personalized treatment plans for orthopedic joint replacements and geriatric patients with multiple comorbidities
- Complete approximately 5 meaningful patient visits each day using the HCHB EMR system
- Travel throughout the service area with tablet provided for seamless documentation
Qualifications
- Current Physical Therapy license in California
- BLS certification required
- Home Health experience preferred but will consider candidates with less than 2 years experience who have Home Health background
- Strong assessment and clinical skills for diverse patient populations
Knowledge & Skills
- Physical Therapy expertise in home health settings
- Experience with orthopedic and geriatric patient care
- Proficiency with HomeCare HomeBase EMR system
- Excellent communication and time management abilities
- Comfortable with independent practice and travel within service area
