Roles & Responsibilities

Deliver personalized physical therapy services in patients' homes across Yolo County
Assess and evaluate each patient's strength, mobility, gait, and rehabilitation potential
Create and adjust individualized treatment plans that help patients regain independence
Educate patients and families on therapeutic exercises and equipment use
Collaborate with physicians and care teams to track progress and adjust goals
Supervise home health aides and coordinate additional support services as needed
Document clinical notes and progress summaries that guide ongoing care

Qualifications

Minimum 2 years of physical therapy experience in acute, home care, or rehab settings
Current BLS certification required
Strong communication and organizational skills
Valid driver's license and reliable transportation for home visits

Knowledge & Skills

Home health physical therapy expertise
Patient assessment and functional evaluation
Rehabilitation program development
Caregiver education and training
Compliance with Medicare and Medi-Cal regulations
