Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Provide compassionate physical therapy services to patients in their homes
Conduct start of care assessments, recertifications, and resumption of care visits
Deliver 5-6 meaningful patient visits per day, making a real difference in each person's recovery journey
Collaborate with a supportive clinical team including physical therapists from sister agencies
Complete OASIS documentation to ensure quality patient outcomes
Perform occasional safety evaluations or teaching visits in hospice situations

Qualifications

Current North Dakota Physical Therapy License required
BLS certification needed
Valid driver's license and auto insurance essential
Home Health experience and OASIS training preferred
First-time travelers welcome to apply

Strong communication and patient education abilities
Ability to work independently while staying connected with the care team
