Candidate Pay Rate: $53 per hour

Roles & Responsibilities

Provide compassionate physical therapy services across four rural counties
Conduct OASIS assessments and Start of Care evaluations
Help patients recover from falls, fractures, and age-related mobility challenges
Manage a manageable caseload of 4-6 patient visits daily
Document visits and track time using provided tablets

Qualifications

Current Physical Therapist license in Indiana
Experience with OASIS assessments and Start of Care documentation
Comfortable working independently in rural settings
Reliable transportation for travel between patient homes

Knowledge & Skills

Strong assessment and clinical reasoning skills
Excellent time management for a point-based visit system
Friendly, professional communication with elderly patients
Ability to adapt to both business casual and hunter green scrubs attire
