Roles & Responsibilities

Evaluate patients' movement, function, and pain levels to develop individualized treatment plans that restore independence and mobility.
Deliver evidence-based therapeutic interventions to reduce pain, improve mobility, and lower fall risk.
Document all aspects of patient care thoroughly — evaluations, progress notes, daily notes, discharges, and insurance communications.
Coordinate discharge planning to ensure safe transitions and optimal functional outcomes for patients.
Communicate clearly and professionally with providers, insurance companies, patients, and rehab team members via phone and written notes.
Support facility operations by assisting with light cleaning and laundry tasks to maintain a safe, welcoming treatment environment.
Collaborate with the rehab team to optimize patient progress and contribute to a positive, team-oriented culture.
Perform other duties as requested to support patient care and clinic needs.

Qualifications

Proven skills in patient evaluation, treatment planning, documentation, discharge planning, mobility improvement, and fall risk mitigation.
Experience providing patient-centered care focused on pain management and functional recovery.
Strong verbal and phone communication skills for coordination with providers and insurance companies.
Comfort with multi-tasking clinical duties and supporting light facility tasks (cleaning, laundry) as part of a team.

Knowledge & Skills

Patient evaluation and progress tracking
Treatment planning and mobility training
Documentation: evaluations, progress notes, discharges, daily notes
Discharge planning and coordination
Fall risk assessment and mitigation strategies
Pain management techniques
Experience interacting with insurance for approvals/denials
Clear phone communication with providers and patients
Team collaboration and supportive clinical workflow
Basic clinic upkeep (cleaning, laundry assistance)
