Candidate Pay Rate: $53 per hour

Roles & Responsibilities

Provide compassionate physical therapy services across four rural counties
Conduct comprehensive Oasis Assessments for accurate patient evaluations
Manage Start of Care cases to establish personalized treatment plans
Deliver 4-6 meaningful patient visits daily using a point-based system
Help elderly patients recover from falls and fractures with expert care
Document visits and track time efficiently using modern tablet technology

Qualifications

Current Physical Therapist license in Indiana
Experience with Oasis Assessments and Start of Care documentation
Comfortable working independently in rural community settings
Strong assessment and clinical reasoning skills

Oasis Assessments expertise
Start of Care case management
Rural healthcare delivery experience
Patient-centered treatment planning
Electronic documentation and time tracking
