Roles & Responsibilities

• Provide skilled physical therapy evaluation, treatment planning, and documentation for inpatient and outpatient patients
• Deliver rehabilitation care across orthopedics, neurological rehab, sub-acute and general inpatient populations
• Collaborate with OT, Speech, nursing and multidisciplinary teams to coordinate safe, effective care
• Perform focused treatments for patients with spinal cord injury, stroke, brain injury, post-op ortho, bariatrics, and ICU patients
• Participate in floor-specific orientation and follow unit protocols and Cerner charting requirements
• Support safe patient transitions between levels of care and communicate progress to the care team and families
• Cover assigned outpatient locations when applicable (Hueme outpatient clinic and Oxnard clinic) with professionalism

Qualifications

• Minimum 2 years of clinical physical therapy experience required
• Active license to practice physical therapy (license to submit required)
• BLS certification required
• Experience using Cerner or similar EMR for documentation
• Comfort and experience working in inpatient rehab, sub-acute, ICU and outpatient orthopedics preferred
• Ability to work required weekends and participate in holiday rotation

Knowledge & Skills

• Inpatient care and acute care rehab workflows
• Outpatient orthopedic and sports medicine therapy
• Neurological rehabilitation (stroke, brain injury, spinal cord injury)
• Sub-acute care and management of long-term patients
• ICU familiarity and working with medically complex patients
• Lymphedema management experience (valuable for outpatient settings)
• Strong charting skills in Cerner
• Team collaboration and effective patient/family communication
